Hmmm. Well, it's sort of an ad hoc fold, really, designed to display the various elements of the design as well as show off the hand rolled edge a bit. This is from memory and not while actually looking at the fold, but lay it out flat then fold up one end enough to get the square into proper...
If you want it to hold a crease, push it into open crown, spritz the top half of it lightly with distilled water then shape it as desired. Let it dry and it should hold the shape fine. I would echo johnnyphi in urging you not to use stiffener.
No pictures allowed in the house, I'm afraid. However, I recall that someone posted a picture of it in an earlier thread on Truman. I'll see if I can dig it up.
