Here's another Stetson Quality Assurance slip that was with a Nutria Quality hat dating from the 1940's to early 50's. John Gavin is the Stetson employee on this slip.
Correction -- I'm not sure that John Gavin Quality Assurance slip came with the Nutria hat but it may have. Here's another slip with Stetson employee Leroy Farley named. The Nutria hat had 1 of these 2 slips in it, behind the sweatband, and another Stetson had the other slip. Also, here's a different kind of slip that came with one of them, I think, but it may have been with some other hats.

Article in The American Hatter, August 1911, about Stetson soon to be opening a store in New York City, about in October 1911. Also a Stetson ad in the same issue of The American Hatter.
