In regards to only the top button being buttoned, I just found this at gentlemansemporium.com, in their Victorian Fashion Guide.

I just looked for a shirt at Goodwill that was a band collar with the outer flap added. A band collar shirt would have saved me a step, but I did not find one in my size.
I pulled the stitches on the collar and then resewed it as a band collar.

You could probably JB Weld a hinge and latch onto a standard ice bucket.
You could also look into a soup or food thermos. They are wider and shorter than a standard thermos.
Someone makes a "travel" ice bucket, but it looks like it may not even have a lid.

The complication I faced was insufficient material. Also, the way mine was cut the tip of the foldover is the widest point. I had to seam to the narrowest points and then cut off the excess. To get the foldover the sleeve would have had to end up much smaller in diameter.
If you are up to a bit of DIY, "How to Make Leather Cases vol. 2" by Al Stohlman (Tandy Leather 61941-02) has instructions for an accordian gusset brief case. There are no straps, but adding them would not be too difficult.
I have the same problem. Extending jacket sleeves is no small task. There is the afore mentioned lining extension and if it has a faux buttoned cuff you will likely have to remove it and convert the sleeve to just seamed. The inside is also more complex than you might expect.
