Seriously, if a custom hat is in your budget, there's no reason not to go custom over an off the rack hat.
I have a Christy's, the color is nice, the block shape is nice, but the ribbon work is shoddy and the felt is not very dense, therefore not very durable.

The size of the flange depends on the width of the brim. As far as the block you use whatever size you want the hat to be, unless you don't want to remove the sweatband, in which case you use one size smaller.
