I knew it was his last movie & I was thinking that he did die from cancer, but I didn't recall how long after The Shootist that death occurred. My memory may differ from the actual facts.Maybe. He didn't die for a few years, but I always assumed it was his way of facing his own mortality.

I think Fruno hit the nail on the head & it may not have had anything to do with the movie, but it is probably fair to say he was facing & accepting his own mortality.The movie was released in theatres in August of 1976; JW died in June of 1979.

"Contrary to popular belief, John Wayne did not have cancer when he made this film. His entire left lung and several ribs had been removed in surgery on September 17, 1964, and in 1969 he was declared cancer-free. It was not until January 12, 1979, almost three years after this movie had been filmed, that the disease was found to have returned. According to a 2014 biography "John Wayne: the Life and Legend" by Scott Eyman, Wayne had been found to have stomach cancer in 1975 but it had gone into remission before filming began on this movie."
Even though he had been declared "cancer free", Mr. Wayne had a number of health issues while filming The Shootist and his doctors were prepared to end his participation in the movie several times as a result. I really liked that movie, too. It was one of his better Westerns, I think. "The Searchers" had to be his best.I really liked "The Shootist",one of his best messages imo.
