I've always thought they came from the days when one size shirt fit all. The shirts all came with extra long sleeves, they'd fit the long armed fellows and the short armed chaps would use the garters to shorten them.
That's a pretty good effort and exactly the way I do mine. I sometimes think it'd be nice to get the knot higher but then on the other hand if you've polished the front stud nicely I think it looks quiet attractive.
I know you said 'off the rack' but you really should check out Natty Shirts, I have several of their shirts and the price isn't far off shop bought and you can design the sort of thing you described.

I often wear a fedora with a leather headband which was a wee bit large when bought so I simply packed a strip or two of felt behind it for a perfect fit. I also wear a flat cap for work which has no leather headband.
