Its a 40 Medium, no alterations, but I find with this fit, it can go either way, reason being, with and without the liner, the fit Dx changes, and also, the Underarm gussets with the Back action, I have no pulling action...my Dx are 173, 5'8", and I range btwn a 40 and 42chest. Also, with this jacket, the Shoulders are slightly longer than the typical 40, which is normally around 18.25", the cut is different...hope it helps...I have room for layering
