Does that mean each customer has their own custom last, or just for each style? I would think having your own last would be in the thousands of dollar shoes.

Ever try to get vintage lsts, or is it easier to just make your own.

As for lasts, I assume my cobbler modifies existing lasts for each customer instead of producing a new last from scratch each time, but the process is entirely "custom for each customer".

Lot's of honkies wore spades too! My grandfather for one. I know a fellow with a couple dozen of deadstock pairs, but they are all sizes five to seven.

To my understanding the spade shape is really more a matter of the sole than the shoe last itself, thus it shouldn't be as difficult to create.
