There's no branding on it that I can tell, but perhaps the oldheads here can identify it.
The only damage is, obviously, to the seat and the underside has a rip in the fabric cloth.

The tear aside, I'm sure all it needs is a good cleaning. It was kept in our basement for years and then moved to the garage over the winter.
